Upcoming Changes to Trellis Health
In early 2026, Trellis Health will transition its Electronic Medical Record (EMR) system and Member Portal from Epic & MyChart to Elation and its Passport portal.
This transition is an important step in moving Trellis closer to the kind of primary care we were created to deliver: more connected, more responsive, and centered on a trusting relationship with your care team.
Most EMRs are designed around insurance billing and administrative workflows. At Trellis, we believe technology should make care more personal and easier to navigate. Moving to Elation and Passport gives us the flexibility to build care around people rather than processes.
This change helps ensure we stay focused on what matters most – knowing you, staying connected between visits, and supporting your health in a way that feels accessible, thoughtful, and human.
What’s Changing
Medical records: Trellis-related medical records and communication will shift to the Elation Passport portal starting in early 2026.
Secure texting: You will be able to engage with your Trellis team via secure SMS without logging into an app, making it easier to get support when you need it.
What Will Not Change
You can expect everything you value about Trellis to remain the same.
Your care team: The same providers and support staff you trust.
Your membership: Unlimited primary care with no billing to insurance.
How you reach us: Phone, message, video, or in person – whatever works best for you.
Your access: Same-day availability and 24/7 support from your Trellis team.
Your continuity of care: Trellis will still help coordinate any care you need outside Trellis, just as we do today.
Your Medical Records and MyChart
You will continue using MyChart for all other MaineHealth care, and all your historical Trellis and MaineHealth records will remain in Epic & MyChart.
Trellis will still have access to the information we need to help guide your care alongside external providers, and your Trellis records will be transferred to Elation.
