What is Trellis Health?

Trellis Health is an advanced primary care program created by MaineHealth to make healthcare simple, connected, and focused on you. Our collaborative care team provides same-day access, longer visits, and 24/7 support so you can stay healthy on your schedule.

How does Trellis Health work?

Trellis operates on a membership model instead of billing insurance. That means one monthly (or annual) membership fee that covers all Trellis care and services – no copays, no surprise bills, and unlimited access to your care team.

What’s included in my membership?

Everything you expect from primary care, and then some! Membership covers annual physicals, sick visits, preventive screenings, chronic care, mental health support, physical therapy, prescription management, lifestyle coaching, and more.

Who will I see at Trellis Health?

You’ll be cared for by your full Trellis care team. Our model isn’t about seeing just one provider. It’s about having the right support at the right time. Your team includes a Primary Care Provider, Physical Therapist, Mental Health Clinician, Pharmacist, Registered Nurse, Health Coach, and Member Services team.

How much does it cost?

Individual membership is $119 per month, but annual and family discounts are available. There are no copays, deductibles, or bills from Trellis. Trellis membership could be sponsored by your employer at no cost to you – be sure to check your benefits materials or contact your HR department to confirm. If you are a Mending Health plan member, your Trellis membership is covered entirely by your plan.

Where are you located?

Our first practice is at 1945 Congress Street, Building C, Suite 101, Portland, ME 04102. We also offer our services virtually, so you can connect with us in whatever way is most convenient and appropriate, whether that’s in person or by video, phone, or message.

We are looking forward to opening our second practice at Cook’s Corner in Brunswick in 2026!

What are your hours? Do you offer after-hours help?

We’re open Monday through Friday, 8:00 AM to 4:30 PM. But we’re here for you anytime. You can call us 24/7 to reach your care team for urgent needs after hours. With same- or next-day appointments, you’ll always be able to get care when you need it.

Do I have to come into the office to get care?

Not always. Developing a trusting relationship with your care team is about getting in touch frequently and easily, and being able to engage in whatever way is most convenient to you. Some things might need an in-person visit, but we’ll always work with your schedule and needs.

Can my family join too?

Yes. Trellis provides care for both adults and children.

What if I need a specialist or outside care?

We’ll help coordinate referrals, schedule appointments, and stay connected with your specialist to keep your care seamless.

Do you provide gynecology or birth control care?

Yes. We offer preventive gynecology services like pap smears and breast exams, plus support for birth control, menopause, and pregnancy counseling.

Do you offer lab tests or minor procedures?

Yes. We do many lab tests right in our office and perform minor procedures like suturing, joint injections, and skin biopsies.

Do you accept insurance?

We don’t bill insurance for our services. Most patients keep their insurance for things like labs, prescriptions, imaging, specialty care, and hospitalization outside Trellis.

How do I join?

Just complete our online form, and our team will follow up to finalize your enrollment and schedule your “Hello!” visit.

What happens during a Hello! visit?

Our member services team will reach out and complete your registration and invite you to schedule our signature Hello! visit. We’ll get to know you, review your past care and health history, talk through your goals, and explain how Trellis works. We’ll also help gather your medical records, identify any labs or referrals you may need, and make sure you feel confident ahead of your new member visit.

What if I already have a doctor? Can I keep them?

When you join Trellis Health, we become your new primary care provider. You can still see other doctors, but your Trellis team will be your main home for care.

Are you accepting new patients?

Yes! Trellis is currently welcoming new members.

How do I contact my team or make an appointment?

You can call us at 207-661-3730, send a message, or email us at careteam@trellishealth.org. Once you’re enrolled, you’ll have 24/7 access to your care team for any needs.
