Marty, Senior Medical Director
Portland, Brunswick
Marty Wesolowski, DO, MBA is the Senior Medical Director at Trellis Health. As a family medicine physician, Marty believes in treating the whole person and knows the best care is delivered with a terrific team. He earned his medical degree at the College of Osteopathic Medicine of the Pacific, completed his residency at St. Anthony’s Hospitals in Denver, and received a Healthcare MBA from the University of Massachusetts. Since 2011, Marty has practiced primary care in Maine and led multiple clinical and operational teams. He is passionate about providing the community with access to excellent, affordable healthcare.
Outside of work, Marty and his wife, also a physician, enjoy Maine’s outdoor adventures year-round with their four children. He prefers to start his day with a run or trail ride, followed by a mug of dark roast.
Allison, Nurse Practitioner
Portland
Allison Talon brings over 20 years of experience to her role as a Family Nurse Practitioner at Trellis Health. Her expertise spans a wide range of care, including sports-related injuries, minor orthopedic procedures, preventive medicine, menopause, and chronic disease management.
Allison earned her Master of Science in Nursing from Simmons Graduate School of Health Studies and holds certifications in Sports Medicine, Adolescent Health, and Transgender Care from UMass Boston. Outside of work, she enjoys photography—capturing moments in sports, with pets, and at birthday parties. Her favorite seasonal coffees are an iced oat chai latte with sweet cream cold foam in summer and a hot vanilla latte in winter.
Molly, Nurse Practitioner
Brunswick
Nearly a decade in cardiology has shaped Molly Montgomery's approach to primary care. As a Nurse Practitioner at Trellis Health's Brunswick practice, Molly draws on her experience in complex chronic disease management and multidisciplinary care to support members' whole health. Empathetic, curious, and always ready with a little humor, she enjoys building relationships and helping members achieve their health goals.
Molly began her nursing career at NYU Langone and Maine Medical Center before joining MaineHealth Cardiology, where she also led a team of more than 35 advanced practice providers. Outside of work, she enjoys sewing her own clothes. Her beverage of choice is Earl Grey tea or a London Fog, never decaf.
Keri, Physical Therapist
Portland
Keri Prince, Doctor of Physical Therapy, is a physical therapist specializing in orthopedic and sports rehabilitation. She empowers Trellis Health members to regain their active lifestyles through a holistic approach to musculoskeletal care. Fluent in Spanish, she embraces cultural competence, ensuring that all patients feel understood and supported.
Keri holds a doctor of physical therapy degree from Duke University School of Medicine and a BA in Global Studies from the University of California, Santa Barbara. A former outdoor guide in Patagonia, Chile, Keri appreciates a good coffee—especially a cortado.
Tricia, Physical Therapist
Brunswick
Tricia Kana, DPT, brings experience in orthopedic rehabilitation, sports medicine, and injury prevention to the Trellis Health care team. A certified running coach with specialized training in dry needling, she enjoys helping people move with confidence, whether they're recovering from an injury, managing chronic pain, or working toward a fitness goal. Her background includes working with patients of all ages and abilities, from competitive runners to rural communities with limited access to care.
Tricia earned her Doctor of Physical Therapy from the University of New England and is a Certified Clinical Athlete Provider.
Morgan, Mental Health Clinician
Portland
Morgan Konisky is a licensed clinical social worker with a deep commitment to holistic, patient-centered care. She believes physical, social, mental, and emotional health are all essential to a person’s well-being. Morgan offers Trellis Health members mental health therapy, connects them with community resources, and collaborates closely with the care team to ensure an integrated approach to health.
For nearly a decade, Morgan has supported individuals managing chronic illnesses, substance use disorders, adjustment issues, grief, anxiety, depression, and acute mental health challenges in settings including in hospice care, psychiatric hospitals, skilled nursing facilities, and primary care clinics.
Morgan earned her BSW and MSW from Colorado State University. Running is her favorite, daily self-care practice. She’ll have a white chocolate mocha, thank you.
Julia, Mental Health Clinician
Brunswick
Julia Bergquist is a licensed clinical social worker whose career has spanned psychiatric emergency departments, primary care clinics, community health, residential housing, and everything in between. At Trellis Health, she brings that breadth to her collaborative role, focused on empowering members, strengthening the mind-body connection, and making mental health support easy to access. Julia holds a MSW from the University of New England and a BA in Psychology from Marymount Manhattan College, where she also minored in photography. She currently serves as adjunct faculty in the University of Southern Maine's Social Work program.
Though she has lived in three different countries, Julia has never once lived inland and has no intention of starting now. She begins each morning with a thimble of black cold brew, then wisely transitions to herbal tea for the rest of the day.
Evelyn, Pharmacist
Portland, Brunswick
Evelyn Olson, Doctor of Pharmacy, is a clinical pharmacist board certified in pharmacotherapy. Passionate about making healthcare more patient-centered, she leads Trellis Health initiatives that empower members to use medications safely and effectively—with a focus on balancing cost, accessibility, and the complexity of treatment regimens.
She holds a BS in Biological Sciences from the University of Alaska Anchorage, a Pharm.D. from the University of Arizona, and an MBA from the University of Alaska Fairbanks. A passionate cook, Evelyn has a love of most food—but don’t serve her black olives! Evelyn's beverage of choice is the Nushagak Fog, a lavender-infused twist on the classic London Fog, named after a river in her rural Alaskan hometown.
Tanja, Registered Nurse and Health Coach
Portland, Brunswick
Tanja Schoenbrod, BSN, RN, CNOR, CHC, is a registered nurse with extensive experience delivering patient-centered care, clinical excellence, and innovative process improvement for Mainers. At Trellis Health, she promotes optimal wellness by combining clinical expertise and patient advocacy, enhancing comprehensive care management of acute and chronic disease, and supporting members to achieve their health goals.
Tanja grew up in Arundel, Maine and earned her BS in nursing from the University of Southern Maine. Among many favorite activities, Tanja can often be found channeling her inner 12-year-old while sailing her Sunfish or savoring a hot latte with whole milk.
Jenelle, Lead Medical Assistant
Portland
Jenelle Wing is a nationally certified medical assistant who seamlessly balances clinical expertise, administrative efficiency, and empathetic leadership. She serves as a vital link between the Trellis Health care team and patients, ensuring all members feel listened to and comfortable as they receive high-quality care.
Jenelle grew up in Maine and earned her certificate in Medical Assisting from Kaplan University. In her free time, she makes park accurate Disney princess gowns and wigs—Rapunzel was her favorite—and attends comic conventions. Her beverage of choice is a dirty chai, extra hot with nutmeg.
Diane, Lead Medical Assistant
Brunswick
Diane Frizzle, CMA, believes good care means listening, advocating for members, and finding ways to say yes. With ten years of experience at Martin's Point Health Care, she has developed a strong foundation in clinical care and an appreciation for the relationships that make healthcare work. As Lead Medical Assistant at Trellis Health, Diane works closely with the care team while helping new members navigate their first steps with Trellis. She particularly enjoys being part of a model where the team has more time to understand and respond to members' needs.
Outside of work, Diane enjoys spending time with her family and pursuing creative projects, especially sewing and DIY home improvements. Drink of choice is a shaken espresso with an extra shot of espresso.
Becky, Medical Assistant and Health Coach
Portland
Becky Richard is a nationally certified medical assistant with 24 years of healthcare experience and a genuine passion for advocating for the people she cares for. Outgoing, determined, and a proud multitasker, she thrives in environments where high-quality, patient-centered care isn't just the goal, it's the standard.
When she's not at work, Becky can usually be found outside. Camping is her happy place, and she'll freely admit that disappearing into the woods with family and friends, and completely losing track of time, is the whole point. She and her husband are raising a full, busy, joyful house of daughters together. Her go-to is a caramel swirl iced coffee with cream.
Rita, Medical Assistant
Portland
Rita Lewis brings more than 30 years of healthcare experience to Trellis Health, including 15 years as a medical assistant. She joined because Trellis felt immediately familiar — a reminder of why she chose healthcare in the first place. Organized, driven, and always eager to learn, Rita believes patient-centered care isn't a philosophy, it's a practice, and she brings that belief to every member interaction.
Outside the clinic, Rita is an enthusiastic explorer of Maine's outdoors by hiking, kayaking, camping, and fishing whenever the weather cooperates. Once the snow arrives, she happily pivots to crocheting and reading. Her beverage of choice is a black tea with two sugars.
Clare, Clinical Assistant and Phlebotomist
Brunswick
With a background spanning clinical care and scientific research, Clare MacDonald, CPT, combines a thorough approach with an optimistic outlook in her role as Medical Assistant at Trellis Health. She spent three years in the University of Maine's Muscle Development Research Lab, where she conducted independent research, and is excited to work in a care model that allows her to build meaningful relationships with members beyond traditional 15-minute appointments.
Clare graduated summa cum laude from the University of Maine with a BS in Biology and is a Certified Phlebotomy Technician. An avid hockey fan who has played for 16 years, Clare loves cheering on her beloved Toronto Maple Leafs. Her Starbucks order is a brown sugar shaken espresso.
Shawn, Member Services Representative
Portland
Shawn Salvador will greet you with a smile and help get you settled in - helping to both support and empower you on your journey. With experience managing multiple wellness facilities, Shawn has seen the correlation between building relationships and care plans with members and the positive outcomes that follow. He is passionate about helping members feel confident, cared for and looking forward to their next Trellis experience.
Shawn is originally from Chicopee, MA holds degrees in Exercise Science from Springfield College and Business Administration from Quinnipiac University. He is a certified Exercise Physiologist, instructs Iyengar Yoga in addition to other fitness classes, and will perform for friends and family spinning fire poi. After a workout Shawn likes to relax with a warm black coffee or hot water with lemon.
Whitney, Member Services Representative
Brunswick
Whitney Libby brings eight years of healthcare experience and a natural talent for connecting people to her role as Member Services Representative at Trellis Health. Drawing on her background at Martin's Point Health Care and in hospitality, she helps members navigate their care and keeps communication between members and their care team running smoothly. Whitney is especially excited to help shape the member experience as Trellis grows.
Whitney attended Southern Maine Community College. Outside of work, she loves trying new recipes and finding creative ways to use what she grows in her garden. The quickest way to win her over? An iced caramel coffee with cream.
Ariele, Member Services Representative
Brunswick
Ariele Griatzky knows that navigating healthcare can be stressful, and she wants every Trellis member to feel comfortable, supported, and heard. With experience in primary care administration, crisis services, and caregiving, she understands the importance of meeting people where they are. What drew her to Trellis was the team's enthusiasm and collaborative spirit, a refreshing approach to healthcare that she's excited to be part of.
Before joining Trellis, Ariele worked at MaineGeneral's Family Medicine Institute and supported individuals experiencing homelessness at an overnight warming center. Outside of work, she's a self-described houseplant lady who loves decorating her home for fall. Her coffee order is iced pumpkin and French vanilla.
